SS-31 (also studied as elamipretide) is a short peptide designed to reach mitochondria. Labs study it because mitochondrial membrane problems show up in many disease models.

What is SS-31?
SS-31 is a tetrapeptide (four amino acids) that can concentrate in mitochondria. A central idea in the literature is that it interacts with cardiolipin, a special fat found in the inner mitochondrial membrane.
Cardiolipin helps keep that membrane organized. When membrane structure is stressed, energy production and oxidative balance can suffer. SS-31 research asks whether stabilizing that environment changes cell outcomes.
What mechanism do papers describe?
Reviews describe SS-31 as:
- Targeting mitochondria rather than acting like a general antioxidant sprinkled everywhere
- Binding or associating with cardiolipin-rich membrane regions
- Supporting membrane organization and bioenergetic readouts in many preclinical models
That is mechanism language. Mechanism papers are not the same as proof that every clinical goal will succeed.
What have clinical programs tested?
SS-31 / elamipretide has been evaluated in multiple clinical research programs. Common study areas include rare mitochondrial disease settings, heart-failure populations, primary mitochondrial myopathy programs, and eye research such as age-related macular degeneration studies.
Results have not been uniform. Some earlier or smaller studies reported signals of interest. Larger trials in certain populations did not meet their primary endpoints. That mixed pattern is important for honest reading of the literature.
How should beginners read mixed trial results?
Mixed results usually mean one of these things:
- The biology is real in some models, but the chosen clinical endpoint was hard to move
- The disease group was too broad for one average effect to show up
- Dose, delivery, or study length did not match the biology well enough
Good research communication keeps both the promising mechanism and the failed endpoints in view.
How does SS-31 compare with MOTS-c research?
Both sit in mitochondrial research conversations, but they are different tools.
